HIPAA Privacy Notice

This is intended for the use and/or disclosure of Protected Health Information (PHI) when providing or seeking treatment, payment, and healthcare operations.

1. This privacy notice contains a thorough and complete description of the uses and/or disclosures of my protected health information (“PHI”) which are necessary to provide me with treatment, and which are also necessary for the Practice to obtain payment for that treatment and to perform other healthcare operations. I have been informed that, upon my request, the privacy notice will be made available to me. Prior to signing this Agreement, the Practice advised me of my right to obtain a copy of the Privacy Notice and has encouraged me to read it in its entirety, in accordance with applicable law.

2. To Protect your privacy and to remain in compliance with applicable law, the Practice reserves the right to change the practices depicted in its Privacy Notice.

3. I am aware that the Practice’s “Notice of Privacy Practices” is displayed in the waiting area and that I am free to request a copy of the same at any time.

4. The Notice of Privacy Practices Contains my rights, as well as the duties and obligations of this office as it relates to my protected health information.

HIPAA policy:

(a) how your company treats Protected Health Information (PHI) or medical records; Lifestyle Health Services is committed to protecting your medical and personal information. We are required by law to maintain the privacy of your Health Information, provide you information about our legal duties and privacy practices, inform you of your rights and the way in which we may use your PHI.

How we may use your PHI

We may use your Health Information about you to provide you with medical services or treatments. We may contact you using your information to remind you of appointments. We may release medical information to an individual you identify involved in your medical care

(b) what the patient’s rights are with regards to protected health information:

In most cases you have the right to view or get a copy of your protected health information (PHI).You have the right to receive a list of instances where we have disclosed your PHI without your written consent for reasons other than treatment or payment.

If you believe that the information in your records is incorrect or if important information is missing, you have the right to request that we correct the existing information or add the missing information. You have the right to limit the use of your sensitive personal information for limited purposes, such as providing you with the services you requested.

California Consumer Privacy Act:

You may request that Lifestyle Health Services disclose to you what personal information they have collected, used, shared, or sold about you, and why they collected, used, shared, or sold that information. Specifically, you may request that businesses disclose:

The categories of personal information collected, Specific pieces of personal information collected, The categories of sources from which the business collected personal information, The purposes for which the business uses the personal information, The categories of third parties with whom the business shares the personal information, The categories of information that the business sells or discloses to third parties.

You may request that Lifestyle Health Services delete personal information they collected from you and to tell their service providers to do the same.

You may request that Lifestyle Health Services stop selling or sharing your personal information (“opt-out”). Note that sharing refers specifically to sharing for cross-context behavioral advertising, which is the targeting of advertising to a consumer based on the consumer’s personal information obtained from the consumer’s online activity across numerous websites. With some exceptions, businesses cannot sell or share your personal information after they receive your opt-out request unless you later provide authorization allowing them to do so again.

Lifestyle Health Services cannot deny goods or services, charge you a different price, or provide a different level or quality of goods or services just because you exercised your rights under the CCPA.

However, if you refuse to provide your personal information to Lifestyle Health Services or ask to delete or stop selling your personal information, and that personal information or sale is necessary for the business to provide you with goods or services, Lifestyle Health Services may not be able to complete that transaction.

Lifestyle Health Services can also offer you promotions, discounts and other deals in exchange for collecting, keeping, or selling your personal information, but can only do this if the financial incentive offered is reasonably related to the value of your personal information. If you ask Lifestyle Health Services to delete or stop selling your personal information, you may not be able to continue participating in the special deals they offer in exchange for personal information. If you are not sure how your request may affect your participation in a special offer, ask Lifestyle Health Services.
